Step 1

To use this service, you need to know your
Hospital No. or Patient ID which is in your Laboratory Order Slip or Admission Form. This will serve as your Username.

Step 2

Your Date of Birth will serve as Password.
Type it in this format:

example: Date of Birth: 9/5/1982
Password: 951982

Step 3

check the I'm not a robot checkbox,
then click the Sign in Button.

Once you've downloaded the lab result, You'll need a PDF Reader to Open the file. If you don't have a PDF Reader, you can download it from the links below.
As of the moment, The result from other test is unavailable online.
